ABA therapy in Alabama

Alabama Medicaid covers ABA for eligible kids, and the state has required private insurance to cover autism care since 2017. Here’s how it actually works — and how we handle the messy parts for you.

Getting approved, in three steps

    An autism diagnosis from a qualified provider is the key that opens coverage. Already have it? You’re ahead. If not, we’ll help you find an evaluation.

    We build and submit the plan

    Your BCBA assesses your child and writes the treatment plan. Then we file it with the right reviewer in Alabama — forms, follow-ups, and all.

    Coverage confirmed. Therapy starts.

    Once the plan signs off, sessions begin. Most families who qualify start within 2–3 weeks. Approvals renew on a schedule — we track it so you never think about it.

Rendering/supervising BCBA submits PA with physician EPSDT Referral Form 362, 97151 assessment report with data/graphs, treatment plan and caregiver training plan; every ABA code except 97151 requires PA, approved in 6-month increments

How many hours a week?

Alabama doesn't set one fixed number — hours are approved child by child. Set case-by-case by medical necessity in 6-month authorization blocks; industry-typical 10-40 hours a week.

Private insurance has to help too

Not on Medicaid? Alabama’s autism insurance law makes state-regulated private plans cover autism care — you’d pay your plan’s normal deductible and copays. The rule on the books: HB284 (2017), effective 10/1/2017, ages 18 and under; annual caps $40K (0-9)/$30K (10-13)/$20K (14-18), but caps/age limit generally unenforced as MHPAEA violations; applies to large-group (51+ employee) plans

Waiver programs worth knowing

Waivers can add services or open Medicaid eligibility even when family income is too high. In Alabama: ID Waiver, Living at Home (LAH) Waiver, Community Waiver Program (CWP) — ADMH-operated, require intellectual-disability/ICF level of care; no autism-specific waiver

What does ABA actually cost in Alabama?

Alabama Medicaid reimburses ABA providers roughly $40 per hour for one-on-one therapy (rates differ by clinician credential and setting). That's what the state pays us — with Medicaid, most families pay nothing out of pocket.
