ABA therapy in Montana
Montana Medicaid covers ABA for eligible kids, and the state has required private insurance to cover autism care since 2009. Here’s how it actually works — and how we handle the messy parts for you.
Does Medicaid cover ABA in Montana? Yes.
THE COVERAGE PATHWAY
Covered as a state-plan/EPSDT benefit for members age 20 and under with ASD (SED to 17); fee-for-service through Montana Healthcare Programs — no MCOs; governed by the Montana Medicaid Applied Behavior Analysis Services Manual
WHO RUNS IT

Getting approved, in three steps
Montana requires paperwork before therapy is covered. Here’s the whole process from your side of the table — we do the rest.
1
Round up the diagnosis paperwork
An autism diagnosis from a qualified provider is the key that opens coverage. Already have it? You’re ahead. If not, we’ll help you find an evaluation.
2
We build and submit the plan
Your BCBA assesses your child and writes the treatment plan. Then we file it with the right reviewer in Montana — forms, follow-ups, and all.
3
Coverage confirmed. Therapy starts.
Once the plan signs off, sessions begin. Most families who qualify start within 2–3 weeks. Approvals renew on a schedule — we track it so you never think about it.
The fine print, if you want it (Montana’s exact rules)
The directing BCBA submits prior authorization: an Intent to Initiate Treatment form plus a physician/licensed mental health professional prescription, diagnostic evaluation, behavior identification assessment, and a treatment plan with quantifiable goals; re-auth via Additional Units of Service Request after the initial 180 days

How many hours a week?
Initial authorization = 1,260 15-min units per 180 days (~315 hours, ~12 hours a week average); more via re-authorization
Private insurance has to help too
Not on Medicaid? Montana’s autism insurance law makes state-regulated private plans cover autism care — you’d pay your plan’s normal deductible and copays. The rule on the books: Enacted 2009 (SB 234, effective 1/1/2010); covers ages 18 and under; statutory caps of $50,000/yr through age 8 and $20,000/yr ages 9-18 remain in MCA 33-22-515 but are generally unenforceable under federal parity (MHPAEA)
Waiver programs worth knowing
Waivers can add services or open Medicaid eligibility even when family income is too high. In Montana: 0208 Comprehensive DD Waiver (Developmental Disabilities Program); Big Sky Waiver; former Children's Autism Waiver ended — ABA moved to the state-plan benefit
What does ABA actually cost in Montana?
Montana Medicaid reimburses ABA providers roughly $45 per hour for one-on-one therapy (rates differ by clinician credential and setting). That's what the state pays us — with Medicaid, most families pay nothing out of pocket.
